Additional documents that you will need to provide us with are:
- Signed Agreement for Professional Services.
- Background Check Applications for each technician/helper that will be servicing our clients/customers. This can also be done online if it will be more convenient. Please tell us which way you would like to submit this.
- W-9 Form for tax purposes.
- EFT Form – Electronic Funds Transfer right into your account!

- Voided Check &ndash You will be paid electronically!!!
- $1,000,000 Liability Insurance Certificate with INSTALLS inc named as additional insured as well as the certificate holder.
- Proof that you carry Workers Comp Insurance.
